Open Enrollment 2018

Welcome to 2018 Open Enrollment!!

This is your one chance to make changes affecting your 2018 benefits by reviewing the latest plan to see what’s new and different, add or drop eligible family members, review your premium contributions and make your selections before the window closes on November 30th.

Medical & Dental:

You have the option to add/remove dependents from your plan effective for the 2018 plan year.  Enrollment must be the same for Medical and Dental. 

If you are not making any changes, no forms are required.

Waiver of Dependent Coverage Incentive:

If you waive coverage on eligible dependents who have other coverage and you are enrolled as a single participant on the City plan, with proof of dependent relationship and proof of other coverage, the City will fund your Health Reimbursement Account (HRA) with the two party/full family HRA amount.  You must re-enroll and submit proof annually at open enrollment to be eligible to receive this incentive.  Completed forms must be signed and may be delivered to HR or emailed to: *

Flexible Spending Account (FSA):

Action is required each year to receive this benefit. The FSA benefit allows you to set aside pre-tax dollars for medical, childcare or other health related premium expenses. Follow instructions on the link below to enroll online or complete the paper form and submit to HR.


Sick Leave Reserve Program (SLRP):

SLRP is available to Non-Union, SEIU and AFSCME employees.  You must enroll annually.  Follow the instructions here to complete your SLRP enrollment for 2018.

Beneficiary Update:

The City and/or benefit carriers keep record of beneficiary designations should an eligible event happen to a covered person. Please fill out new forms if you have not made updates recently or if you do not know who your current beneficiaries are.  You must complete a new form for each carrier. Completed forms must be signed and may be delivered to HR or emailed to: *

Life Insurance: Hartford is our current life insurance carrier.  All eligible employees have City paid life insurance and many employees elect additional voluntary coverage.  All eligible employees should have a designation of beneficiary on file.
